Care Plan

Annual Membership

Rockford Ambulance offers an annual membership program called the Care Plan to the residents in our service area. For only $39 per year, you and your family will be covered from any out of pocket expenses incurred from medically necessary ambulance transports to and from the hospital.

Health care costs continue to rise at alarming rates along with increased co-pays and deductibles. Joining the Care Plan will eliminate out of pocket expenses for medically necessary ambulance transports.

Service Area

Rockford Ambulance provides coverage to residents in the cities of Rockford, Cedar Springs, and Lowell; in the villages of Sparta, Sand Lake, Belmont, and Kent City; and in the Kent County townships of Algoma, Alpine, Cannon, Courtland, Grand Rapids Charter, Grattan, Lowell, Nelson, Oakfield, Plainfield, Solon, Sparta, Spencer, Tyrone and Vergennes. We also service the residents of Chester Township in Ottawa County.

Wheelchair Transportation Service

Rockford Ambulance offers wheelchair transportation services to the residents of our area. The wheelchair services are perfect for going to and from the doctor’s appointments, dental appointment or just to get out of the house. The Care Plan does not cover the wheelchair services; however, our members are offered the service at a discounted rate.
